Enfrentamiento quirúrgigo en la reconstrucción funcional de la extremidad superior, en pacientes terapléjicos

ResumenLos pacientes con lesión medular, hoy no tienen tratamiento, y cuando tienen lesiones altas (tetraplejia), quedan expuestos a una gran inhabilidad de función de sus extremidades superiores. Recuperar la funcionalidad de estas, aparece como una de las más grandes demandas de estos pacientes. Varias han sido las propuestas quirúrgicas en el tiempo para devolver la función de la extremidad superior. Actualmente, se privilegia hacer, en un acto quirúrgico, la reanimación de la función de pinza de la mano, la que se ve potenciada en forma significativa, si hay una buena función de la flexión y extensión del codo. De no existir extensión de codo, ésta debe ser reconstruida también. Las mediciones de los resultados se centran en evaluar la satisfacción y desempeño de actividades, que son identificadas por los pacientes como prioritarias. Nuestra experiencia de tres años en Teletón Santiago, nos muestran resultados alentadores con esta propuesta de tratamiento.

SummaryFor patients with spinal cord injury there is no available treatment, and when they have upper level injury (tetraplegia), they are exposed to great inability to function in their upper limbs. Recovery of the function of these, appears to be one of the greatest demands among these patients. Many surgical protocols have been proposed to restore upper extremity function. Currently, it is favored, within one surgery, to resuscitate the hand grip function, which significantly improves if there is a good function of elbow flexion and extension. If there is no elbow extension, this movement must also be rebuilt.The outcome measures focused on assessing satisfaction and performance of activities that are identified as priorities by the patients. Our three years experience in Teletón Santiago, show encouraging results with this treatment approach.
