RésuméLes troubles de la statique pelvienne altèrent souvent la qualité de vie des malades du fait des symptômes qu’ils entraînent, incluant une constipation distale, une incontinence anale, des douleurs anorectales. La défécographie conventionnelle et l’IRM–défécographie peuvent identifier une rectocèle, une procidence rectale, une entérocèle, une sigmoïdocèle avec une prévalence élevée chez les femmes souffrant de constipation distale, d’incontinence anale et de douleur pelvienne chronique. Dans cet article nous discutons les troubles de la statique périnéale postérieure qu’il faut rechercher à l’origine d’une constipation distale, d’une incontinence anale ou d’une douleur anopérinéale. La place de la défécographie par rapport à l’examen clinique pour le diagnostic de ces troubles de la statique périnéale postérieure est discutée en rappelant que 50 % des entérocèles et 100 % des sigmoïdocèles sont méconnues par l’examen clinique.
Pelvic floor abnormalities often impact significantly the quality of life and result in a variety of symptoms, including chronic pelvic pain, fecal incontinence, and obstructed constipation. Fluoroscopic defecography and MR defecography enable identification of rectocele, rectal prolapse, enterocele, sigmoidocele with high prevalence in female patients with obstructed constipation, fecal incontinence, and chronic pelvic pain. In this manuscript, we describe the techniques and indications of the two techniques of defecography. We discuss the abnormalities of the posterior pelvic floor compartment at the origin of constipation, incontinence, chronic pelvic pain. Finally we compare the data obtained by clinical examination and defecography, remembering that 50% of enterocele and 100% of sigmoidocele are missed at clinical examination.
