PurposeTo determine whether extreme pseudophakic monovision can reduce or eliminate diplopia in patients with cataract and longstanding acquired strabismus.SettingDepartment of Ophthalmology, University of Cincinnati, and the Cincinnati Eye Institute, Cincinnati, Ohio, USA.DesignCase series.MethodsIntentional extreme monovision was created in patients with stable diplopia having cataract surgery. Intraocular lens selection was targeted for emmetropia in 1 eye and at least 3.0 diopters of myopia in the fellow eye.ResultsTwelve patients with stable diplopia attained excellent uncorrected distance and near vision with a marked reduction in or elimination of double vision.ConclusionPatients with stable acquired strabismus with diplopia may be candidates for extreme pseudophakic monovision, which may be a new strategy to eliminate double vision.Financial DisclosureNo author has a financial or proprietary interest in any material or method mentioned.
