The current coordination structures are multiple and varied. The return to home and the home care services are segmented by pathology, patient profile, and geographic areas. Based on experience analysis and field evaluation of professionals, patients and families's needs, the Support Unit to Home Return of Santelys works transversal and across organisations. Its objective: to guide and coordinate patients' return to home and homecare services whatever their age, their health conditions, and their level of dependence. This system allows to make the health care pathway more smooth while avoiding breaks, especially in more complex cases. The Support Unit to Home Return of Santelys is in line with the guidelines driven by the modernization law of the French health system in terms of coordination improvements of health care pathway.
