Diffusion des Tests de Diagnostic Rapide et organisation des soins de premier recours. Quelles conséquences ? À propos du TDR angine

Technological advances in diagnostic tests and communication systems could gradually change the organization of primary care. The French reforms in 2009 and 2010 invite to study the hypothesis of use by community pharmacists of rapid diagnostic tests for strep throat. This perspective appears to be of major interest for the orientation of people who consult few or not. It requires a specific authorization, clinical formations and adapted incentives, and especially a strong local cooperation between physicians and pharmacists for optimizing patient's orientation and care. It also requires elaboration of strict protocols in order to avoid the jeopardization of healthcare quality, to prevent clinical complications and the misuse of antibiotherapies. The hypothesis studied highlights some key points of method that can be extrapolated to the use of other rapid diagnostic tests and to their categorization, in the multidisciplinary search for quality, equity and cost-effectiveness of primary care, in a ever more constraining environnement (sept 2011).
