Evaluation of the frequency of dispensing electronically discontinued medications and associated outcomes

Electronic prescriptions that have been discontinued within the EHR continue to be dispensed to patients in the outpatient pharmacy setting. These inadvertently dispensed prescriptions have the potential to cause patient harm.
