The patient, aged 13, had already had treatment with a removable appliance; she displayed a severe dental and skeletal Class III but declined surgery. Growth forecasts showed a posterior rotational tendency. The issue of the advanced bone age and shortage of residual maxillary growth were discussed. The patient was forewarned of the possible persistence of a post-treatment prognathic profile. Orthodontic treatment was commenced with extraction of 15-25-34-44 and preparation of maximum anchorage.
