Performances analytique et clinique des dosages d'anticorps antithyroperoxydase réalisés sur quatre automates d'immunoanalyse dans l'exploration des maladies thyroïdiennes auto-immunes

In this work, the analytical and clinical performances of four automated antithyroperoxidase antibodies (TPOAb) assays were studied in 234 patients with autoimmune thyroid diseases. The results were compared to those obtained with a radioimmunoassay (RIA). The precision found with automated immunoassays was higher than that obtained with RIA. For the ACS:180® and the Advia-Centaur®, dilution curves did not give straight lines in high concentrations. Using the cut-off given by the manufacturers, the concordance, the control values and the clinical sensitivity in the exploration of autoimmune thyroid diseases were similar for the l'ACS:180®, the Advia-Centaur® and the Kryptor® as those obtained with RIA. TPOAbs measured on the Immulite 2000® gave similar results in autoimmune thyroiditis but a higher prevalence in Graves'disease. A cut-off of 50 kUI/L gave a better specificity without decreasing the clinical sensitivity. Because of their rapid implementation, the automated TPOAb assays may advantageously replace RIA in the exploration of thyroid autoimmune diseases.
