► We systematically review the literature on exposure therapy in eating disorders. ► We make recommendations to improve exposure therapy in clinical practice. ► We use knowledge from learning theory, neuroscience, pharmacology, and virtual reality. ► Preparatory work and optimised protocols may improve exposure therapy. ► Further advances may come from pharmacological or neuromodulation as adjuncts to exposure.
