Kernelemente dieses KoPM-Modells sind eine gemeinsam gelebte Patientenorientierung, ein Prozessorganisations-Ansatz in der Versorgung, eine gelingende Kommunikation sowie eine die Prozesse unterstützende Aufbaustruktur. Eine Weiterentwicklung dieses Ansatzes durch Effizienzsteigerung der gemeinsamen Kommunikation, durch Qualifizierungsprozesse und durch eine Evaluation von Kooperationsprojekten sind die weiteren Aufgaben.
