Surveillance après radiothérapie stéréotaxique des tumeurs pulmonaires

RésuméActuellement, la radiothérapie stéréotaxique pulmonaire est une alternative dans le traitement à visée curative des tumeurs pulmonaires inopérables. Les modifications scanographiques après radiothérapie stéréotaxique sont fréquentes et peuvent masquer une rechute locale. Comparativement à une irradiation classique, l’irradiation stéréotaxique entraîne des modifications radiologiques différentes. Il apparaît donc nécessaire de standardiser les critères d’évaluation cliniques, radiologiques et fonctionnels afin de mieux différencier une éventuelle rechute d’une pneumopathie interstitielle radio-induite. Cet article propose une revue de la littérature des différentes modifications radiologiques observées après radiothérapie stéréotaxique de tumeurs pulmonaires et des différentes modalités d’évaluation disponibles, notamment la place de la tomoscintigraphie par émission de positons-scanographie au (18F)-fluorodésoxyglucose. Nous proposons un algorithme de surveillance après radiothérapie stéréotaxique pulmonaire intégrant ces différents examens.

Nowadays, bronchopulmonary stereotactic body radiotherapy is recommended as the standard of care in treating medically inoperable patients with curative intent according to guidelines of multiple organizations. Changes in lung density on computed tomography are common after stereotactic body radiotherapy and can confound the early detection of recurrence. Radiation-induced computed tomography lung changes after bronchopulmonary stereotactic body radiotherapy differ from those observed after conventionally fractionated radiotherapy. It is mandatory to standardize and simplify our clinical, radiological and functional evaluation criteria to better distinguish tumor progression and radiation-induced pneumonitis. We performed a literature review to describe post-stereotactic body radiotherapy findings on computed tomography and positron emission tomography, identify imaging characteristics that predict recurrence and propose a follow-up algorithm.
