Technical advancements in imaging, in radiation therapy (RT) planning and RT delivery, have facilitated the safe delivery of conformal radiation therapy to patients with unresectable hepatocellular carcinoma (HCC). Although experience in liver cancer RT is limited, the RT technologies and tools to deliver RT safely are being disseminated rapidly. A variety of doses and RT fractionations have been used to treat HCC, and RT has been used in combination with other therapies including transarterial hepatic chemoembolization (TACE). Outcomes following RT alone or RT and TACE appear better than outcomes following similar historical controls of TACE alone, however, randomized trials of RT are needed. The first site of recurrence following RT is most often within the liver, away from the high dose volume, providing rationale for combining RT with regional or systemic therapies. Given the vascular properties of HCC, the combination of RT with anti-VEGF targeted agents may improve outcomes further.
RésuméLes avancées de l’imagerie, qu’il s’agisse de la préparation ou de la réalisation, ont facilité la délivrance d’une radiothérapie conformationelle dans le carcinome hépatocellulaire. Quoique l’expérience accumulée de radiothérapie hépatique soit limitée, les technologies et les outils requis pour délivrer la radiothérapie en sécurité ont été rapidement mis au point. Diverse doses et fractionnements ont été utilisés pour irradier le carcinome hépatocellulaire et la radiothérapie été souvent utilisée en combinaison avec d’autres thérapeutiques, comme la chémo-embolisation artérielle hépatique. Les résultats obtenus avec la radiothérapie, exclusive ou en combinaison avec la chémo-embolisation, semblent meilleurs que ceux de la chémo-embolisation exclusive, mais des essais randomisés sont nécessaires. Le premier site de récidive après la radiothérapie est le plus souvent hépatique, à distance du volume irradié à forte dose, ce qui incite à proposer d’associer la radiothérapie et une thérapeutique régionale ou systémique. Étant donné la vascularisation du carcinome hépatocellulaire, la combinaison de la radiothérapie et d’une thérapeutique ciblée anti-VEGF pourrait aussi améliorer les résultats.
