Intensive care research is nourished by multiple sources and should not be restricted by rigid schemes. Knowledge gained from all kinds of experiments including basic research must be incorporated with clinical pathophysiological approaches using modern methodological tools. Randomized controlled trials are far from being the only response to these questions. In the field of acute respiratory distress syndrome, the place of noninvasive ventilation, the need for spontaneous breathing during invasive mechanical ventilation and a protective approach towards pulmonary circulation are priorities for clinical research.
