Current physiological data support the use of non-invasive ventilation during acute hypoxemic respiratory failure, even if controversy exists regarding the different indications. Whatever acute respiratory failure type (even in patients with cardiogenic pulmonary oedema), several physiological arguments do suggest a superiority of pressure support plus positive end expiratory pressure over CPAP alone in terms of a better clinical improvement and optimal work of breathing decrease.
