RésuméUn homme de 45 ans a présenté une atteinte phrénique bilatérale dans un contexte de médiastinite nécrosante descendante, nécessitant une assistance ventilatoire continue sur trachéotomie. Les kinésithérapeutes ont tenté de faciliter le recrutement du diaphragme par des réglages de ventilation adaptés, au repos comme à l’effort, en limitant la sur-assistance ventilatoire.
