The autonomous nervous system regulates function of nutrition and homeostasis and prepares the organism for emergency situations. The parasympathetic system is organized around the cranial nerves (particularly the X) which are relayed in the visceral plexuses before reaching the viscera. The sympathetic system is organized around sympathetic centers situated in the spinal cord from metamere C8 to L2. This system vehicles nociceptive input via the communicating white branches to the spinal cord centers where the input can be transmitted to the somatic nervous system by the communicating gray branches.Studies have demonstrated the richness of communicating white branches in the L1 and L2 metameres which would tend to confirm the integration at this level of sympathetic inflow from more distal regions. Knowledge of this organization allows a double approach to patients with lower back pain.Level of evidence:not applicable
