Die Fascia thoracolumbalis als potenzielle Ursache für Rückenschmerzen: anatomische Grundlagen und klinische Aspekte

ZusammenfassungDie Fascia thoracolumbalis (FTL) ist die größte Aponeurose des menschlichen Körpers und besitzt zahlreiche biomechanische und neurophysiologische Aufgaben. Und obwohl diese Struktur die Physiologie des Halteapparates bedeutend mitbestimmt, gibt es bisher nur wenige systematische Untersuchungen hinsichtlich ihrer Bedeutung bei der Entstehung und Aufrechterhaltung chronischer Rückenschmerzen.Im Folgenden wird beschrieben, dass die Fascia thoracolumbalis wichtige anatomische und physiologische Eigenschaften besitzt, und ihre Relevanz für den klinischen Alltag wird kurz dargestellt. Dabei wird gezeigt, dass die Fascia thoracolumbalis sowohl direkt als auch als indirekt an der Entstehung und Aufrechterhaltung chronischer Rückenschmerzen beteiligt sein kann. Die Fascia thoracolumbalis sollte daher bei den differenzialdiagnostischen und therapeutischen Überlegungen von chronischen Rückenschmerzsyndromen immer mit in Betracht gezogen werden.

Even though the thoracolumbar fascia (TLF) is the most extensive aponeurosis in humans and is characterized by various biomechanical as well as neurophysiological properties, there are only few systematic analysis regarding the role of the TLF in the development and maintenance of chronic low back pain. The anatomical and physiological characteristics of the TLF are clearly presented and the clinical relevance is discussed. It is demonstrated that the TLF can be responsible for the development and maintenance of chronic back pain directly and indirectly. Thus, the TLF should be always taken into account in diagnostic and therapeutic considerations regarding low back pain.
