Progress in anti-retroviral treatments now offers hope for HIV-infected persons who want to found a family. In France, since 2001 regulations allow the use of medically assisted reproduction techniques in order to reduce the risk of viral transmission between partners or to treat infertility. For this, it is indispensable to control the HIV infection and any HBV or HCV co-infections and to ensure psychological support for the couple. When the male partner is infected, medically assisted reproduction is the only way to enable conception while maintaining the couple's protected sexual relations. The ejaculate is concentrated with search for HIV in the final spermatozoid fraction. Frozen samples are validated before use. When the female partner is infected, the couple must be informed of the risk of transmission of HIV infection to the infant. Treatment should be optimized to reduce the risk. Self-administered inseminations can be proposed after an infertility work-up in women aged over 38 years. The project to have a child should be managed rapidly so medically assisted reproduction can be proposed in the event of unsuccessful fertilization without medical assistance; ovarian function appears to decline more rapidly in infected women. If the ovarian function is normal, the chances of success are the same as for the general population. Twin pregnancies should be avoided because of the increased risk of maternofetal transmission of HIV. Several thousand couples in Europe have used medically assisted reproduction with no reported case of fetal contamination. The expected success rate is about 50%.
