M.B., a 62-year-old man, showed involuntary and uncontrolled tears due to a right cerebellar ischemic stroke. The analysis of these symptoms and its distinction from a mood disorder allow us to diagnose an involuntary emotional expression disorder. This specific caring has two components: an educational and medicinal approach. The patient and his family circle were informed about the neurologic etiology of the tears episodes. Sertraline was initiated, the efficacy of the selective serotonin reuptake inhibitors is documented for many years. This treatment has shown to be effective after only 3 days. This made the involuntary and uncontrolled tears disappeare after 7 days. The dysregulation of emotional expression are common after brain damage. The understanding and the diagnosis of the involuntary emotional expression disorder are parts of caring in physical medicine and rehabilitation. The selective serotonin reuptake inhibitors are first-line pharmacotherapy for these symptoms.
