SummaryTypical reasons for failed Hallux valgus surgery include the reoccurrence of the deformity, Hallux varus, irritations by ostheosythesis material, increasing degenerative arthritis of the metatarsophalangeal joint, avascular necrosis of the metatarsal head, transfer metatarsalgia, non union, wound problems and infections, CRPS (Complex regional pain Syndrome) or a combination of the mentioned pathologies. The proactive approach to the problems includes imaging (CT and MRI), the consultation of other disciplines and if indicated the fast decision for revision surgery. If malalignment of the first ray is the problem, it should be analyzed if the operative method of the primary surgery was probably not adequate for the problem, or if the method in principal was correct, however it was a technical fault which led to the unsatisfactory result. If it was the wrong concept, the right method should be chosen in the revision, if it was a poor technique, it should be redone in the right way.In patients with destruction of the first metatarsophalangeal joint, the fusion is the most reliable concept. The potential of hardware removal is often underestimated.
