Minimally-invasive alternatives in the treatment of distal articular tibial fractures

Distal tibial fractures constitute a wide spectrum of lesions from simple metaphyseal and diaphyseal fractures to methaphyseal collapse with articular surface impaction and severe compromise of the soft tissue. The standard of care still remains controversial. This review discusses the indications for minimally-invasive techniques based on fracture pathoanatomy. A surgical approach is proposed, reserving arthoscopically-assisted reduction of the joint surface for particular cases. A literature review is conducted, describing surgical outcomes and complications.

ZusammenfassungDistale Tibiafrakturen weisen ein breites Spektrum von einfachen metaphysären und diaphysären Frakturen bis hin zum metapysären Kollaps mit Impression der Gelenkfläche und schwerem begleitendem Weichteilschaden auf. Der Theapiestandard wird weiterhin kontrovers diskutiert. Diese Übersichtsarbeit beschreibt die grundlegenden Prinzipien der minimal-invasiven Behandlungskonzepte in Abhängigkeit von der Frakturmorphologie. Ein operativer Zugang mit arthroskopisch gestützter Gelenkreposition in speziellen Fällen wird vorgestellt. Eine aktuelle Literaturübersicht befasst sich schwerpunktmäßig mit Komplikationen und funktionellen Behandlungsergebnissen dieser Technik.
