Therapie chondraler und osteochondraler Defekte am Talus durch Autologe Matrix Induzierte Chondrogenese

ZusammenfassungDie Behandlung von osteochondralen Defekten des Talus wird unverändert kontrovers diskutiert. Neben dem Débridement des Defekts mit nachfolgender Mikrofrakurierung oder der Transplantation eines osteochondralen Zylinders (OATS) hat sich in den letzten Jahren die Deckung des Defekts mit Kollagenmembranen etabliert. Der Eingriff kann in offener Technik oder arthroskopisch durchgeführt werden. Der Vorteil des offenen Vorgehens ist die Möglichkeit, die Kollagenmembran präzise in den Defekt einzupassen und auch subchondrale Zysten mit einer Spongiosaplastik aufzufüllen. Die arthroskopische Technik bietet den Vorteil einer deutlich geringeren Weichteiltraumatisierung. Nachteilig sind dabei allerdings die limitierten Möglichkeiten der Defektauffüllung.Ziel des Artikels ist es, die offene und arthroskopische Technik der Autologen Matrix Induzierten Chondrogenese (AMIC) in der Therapie von chondralen und osteochondralen Läsionen des Talus mit ersten Ergebnissen darzustellen.

SummaryThe treatment of osteochondral defects of the talus is still controversial. Besides debridement of the defect with microfracture or osteochondral cylinder transplantation (OCT), the coverage of the defect with a collagen membrane has gained increasing popularity. This procedure can either be done open or arthroscopically. The advantage of an open procedure is the precise placement of the membrane and the option to address subchondral cysts with a graft. Advantageous of the arthroscopic approach is fewer trauma to the soft tissues. However, there are technical limitations to fill bony defects with a graft.The aim of this article is to present the open and arthroscopic approach with preliminary results in the treatment of chondral and osteochondral defects of the talus by Autologous Matrix Induced Chondrogenesis (AMIC).
