The flexible flat foot of the child is at least until the end of the age of five a physiological form of normal growth, apart from special entities like talocalcaneal coalitions or the flat foot as an epiphenomenon of ICP. Only 1 -2% will persist. Then the development of a fixed form associated with symptoms is possible. The subtalar arthroereisis with an endorthesis which is fixed in the sinus tarsi is a minimally invasive surgical treatment to correct the deformity. In the period from 3 / 2008 to 9 / 2009 a total of 16 subtalar arthroereisis were performed in 12 children. The surgical treatment followed in all cases an unsuccessful conservative therapy with the deformity persisting or even increasing over years. Every child suffered from a flexible flat foot. Through the operation the shape of the foot could be improved. Thus, a correction of the average talocalcaneal angle from 145° preoperatively to 125° postoperatively, of Kite's angle from 40° to 27° and the talometatarsal angle from18° to 7° was possible. One endorthesis dislocated and had to be removed. Overall, satisfactory results could be achieved in 94%. The subtalar arthroereisis by means of an endorthesis fixed in the sinus tarsi is a minimal invasive procedure that in the case of displacement or inadequate correction also provides plenty of withdrawal options such as bone osteotomies.
