Extraskelettales myxoides Chondrosarkom – Manifestation am Fuß

ZusammenfassungEs wird über den seltenen Fall eines extraskelettalen myxoiden Chondrosarkoms am Fuß berichtet. Die betroffene Patientin stellte sich wegen einer an Größe langsam zunehmenden nicht schmerzhaften Schwellung im 4. Intermetatarsalraum unter der Primärdiagnose eines Ganglions zur Excision vor. Histologisch konnte eine extraskelettale Manifestation eins myxoiden Chondrosarkoms beschrieben werden. Anhand der Literatur wird auf die Besonderheiten dieser seltenen Neubildung eingegangen. Da bei extraskelettalen Chondrosarkomen mit einer Rezidivrate von bis zu 50% gerechnet werden muss und sie zudem meist weder strahlensensibel noch einer Chemotherapie zugängig sind, muss die primäre Resektion den Tumor komplett entfernen. Eine konsequente Nachsorge in Zusammenarbeit mit einem onkologischen Zentrum sollte gewährleistet sein. In der überwiegenden Zahl der Fälle sind tumoröse Neubildungen am Fuß benigner Natur. Dennoch sollten Resektionen nach Möglichkeit den Prozess en bloc entfernen, um die Therapiechancen bei überraschend bösartigen Befunden nicht zu mindern.

SummaryA 39 year old woman presented with a slowly growing painless swelling in the 4th intermetatarsal space in the right foot. The resection was done under the primary diagnosis of a ganglion cyst. Surprisingly, the histology gave evidence of an extraskeletal myxoid chondrosarcoma. A comprehensive analysis of the literature on these rare tumors in the extremities revealed that there is a predilection for middle-aged men (male: female ratio, approximately 2:1), especially in the lower extremity. The history of the presented case is typical with a slowly growing painless soft-tissue mass with varying degrees of radiopacity. The in situ appearance was that of an encapsulated lobulated tumor with no obvious sign of bony erosion or infiltration in the surrounding structures. As the overall recurrence rate of extraskelattal chondrosarcomas is about 50% and radiosensitivity as adjuvant chemotherapy is rarely of beneficial effect, en bloc resection is the treatment of choice for the primary tumor. Moreover, due to possible late recurrence of these neoplasms a thorough follow-up of the patient is essential.
