Kallusdistraktion mit intramedullärer Schienung zur Verlängerung des vierten Strahls bei Brachymetatarsie

SummaryBrachymetatarsia is characterized by an abnormal short third or fourth metatarsal. This uncommon condition is usually asymptomatic. We present three case reports demonstrating the use of distraction lengthening for symptomatic congenital short metatarsals. In all three cases the distraction lengthening of the fourth metatarsal was performed with an external fixateur system aided by an intramedullary K-wire without any supplemental bone graft. This technique allows complete correction of the difference in toe length without any axial deviation. All patients have an excellent clinical and radiological correction with no evidence of recurrent transfer lesions one year after operation. A high degree of patient and family compliance, as well as the information about the long treatment, is necessary to obtain a successful outcome for this procedure.

ZusammenfassungDie Brachymetatarsie beschreibt die Verkürzung des dritten und/oder vierten Mittelfußknochens. Diese relativ seltene Erkrankung ist in der Regel asymptomatisch. Wir berichten über die isolierte Kallusdistraktion des vierten Strahls in drei Fällen einer symptomatischen Brachymetatarsie mit einem externen Fixateur-System und intramedullärer Drahtschienung ohne die Anlagerung von Knochen. Mit dieser Methode kann ein vollständiger Längenausgleich ohne Achsabweichung des Zehenstrahls erreicht werden. Bei der Nachuntersuchung ein Jahr nach der Operation zeigten sich sehr gute klinische und radiologische Ergebnisse. Eine hohe Compliance von Patient und Familie, sowie eine Aufklärung über die Behandlungsdauer, sind für eine erfolgreiche Therapie notwendig.
