À propos d’un syndrome de Lemierre secondaire à une parotidite

RésuméLe syndrome de Lemierre se caractérise par une infection oropharyngée première suivie d’une thrombophlébite veineuse jugulaire interne et enfin d’une dissémination métastatique septique. Fusobacterium necrophorum est l’agent pathogène le plus souvent à l’origine de cette nécrobacillose. Le diagnostic s’appuie tout d’abord sur la clinique, puis la microbiologie avec les hémocultures en première ligne et enfin l’imagerie avec la tomodensitométrie. La prise en charge quant à elle repose sur l’antibiothérapie dirigée contre les germes anaérobies et les bacilles à Gram négatif à laquelle peut s’ajouter un traitement complémentaire chirurgical. Nous rapportons ici le cas d’un patient de 73 ans présentant un tableau peu fréquent de ce syndrome puisque le point de départ infectieux est une parotidite suppurée associée à un abcès rétropharyngé. Ce cas est d’autant plus rare qu’il semble être le premier rapporté dans la littérature concernant un syndrome de Lemierre à F. necrophorum sur parotidite.

Lemierre's syndrome is a classical presentation of human necrobacillosis. It is characterized by a primary infection in the face including a septic thrombophlebitis of the internal jugular vein and disseminated metastatic abcesses. Fusobacterium necrophorum is the main pathogen found in that syndrome. The diagnosis is based on clinical features, then on the microbiology with positive anaerobic blood cultures as key role and finally on the computed tomography. Most of the time a well-chosen antibiotic treatment against anaerobic pathogens and Gram negative bacilli is efficient but surgery can be useful. We report a case of a 73 years old man, which seems to be unique because it is the first case reported of a Lemierre's syndrome characterized by a parotitis infected by F. necrophorum.
