Épiglottite aiguë obstructive en réanimation : stratégie pour une intubation à haut risque

RésuméL’épiglottite aiguë est une infection des structures supraglottiques qui s’accompagne d’un risque vital potentiel par arrêt respiratoire obstructif brutal. Ce cas clinique rapporte la prise en charge extrahospitalière puis en réanimation d’un patient victime d’une détresse respiratoire imputable à une épiglottite aiguë obstructive et ayant justifié une réanimation cardiorespiratoire initiale. L’extubation accidentelle précoce du patient et l’aggravation de la dyspnée laryngée ont imposé une ré-intubation en urgence associant une intubation en ventilation spontanée guidée par nasofibroscopie sous couvert d’une préparation de ventilation transtrachéale percutanée. Ce cas clinique expose la stratégie de contrôle des voies aériennes utilisée avec succès pour ce patient.

Acute epiglottitis is an infection of supraglottic structures, with a high risk of life-threatening respiratory obstruction. We report a case of a 52-year-old patient who presented with a prehospital hypoxic respiratory arrest caused by obstructive oedema with a successful resuscitation. Laryngoscopy after a sudden unplanned extubation permits diagnosis of acute obstructive epiglottitis. The growing threat of laryngeal dyspnea prompts emergency tracheal airway protection, by means of the combined preparation of transtracheal oxygenation access and the use of fiber-optic laryngoscopy. This case report emphasises the airway management strategy successfully used in this patient.
