Hémorragie massive au cours d’un traumatisme craniofacial traitée par ligature de la carotide externe

RésuméLes hémorragies faciales massives au cours des traumatismes craniofaciaux sont rares. La richesse de la circulation collatérale entre les carotides internes et externes explique les difficultés d’hémostase. Nous rapportons le cas d’un homme de 63 ans qui a présenté une hémorragie faciale massive associée à un traumatisme crânien grave avec hématome sous-dural imposant une évacuation chirurgicale. Bien que l’on dispose d’un plateau de radiologie interventionnelle, l’hémostase faciale a été réalisée par une ligature unilatérale de la carotide externe. Trois raisons ont motivé notre décision : hémorragie pluriorificielle excluant des gestes endovasculaires hypersélectifs, efficacité et rapidité de la ligature de la carotide externe, unité de lieu pour cette chirurgie céphalique en deux temps (cervicofaciale et neurochirurgicale). La place de ce moyen d’hémostase dans la prise en charge des hémorragies faciales massives est discutée.

Life-threatening bleeding following craniofacial injury is rare. The rich collateral circulation between external and internal carotid arteries complicates the problem of controlling haemorrhage. We report the case of a 63-year-old man who presented a life-threatening haemorrhage following maxillofacial trauma and a severe head injury with subdural haematoma requiring neurosurgical evacuation. Although we had an interventional radiology unit, facial haemostasis was achieved by an unilateral ligation of the external carotid artery. There were three reasons behind our decision: multiple bleeding excluding selective endovascular embolization, efficiency and speed of the ligation of the external carotid, only one place for this cephalic surgery in two phases (neurosurgery and neck surgery). The role of ligation of the external carotid artery for life-threatening bleeding following facial injury is discussed.
