Blood patch, il n'est jamais trop tard

RésuméLa survenue de céphalées posturales durables après effraction dure-méro-arachnoïdienne (EDA) lors d'une anesthésie péridurale (APD) ou après ponction lombaire est une complication dont l'incidence est devenue faible. En cas d'effraction volontaire, l'usage généralisé, y compris en radiologie, d'aiguilles de faible calibre à pointe non tranchante et la ponction en plaçant sagittalement le biseau de l'aiguille de Tuohy en cas d'APD en ont diminué l'incidence et la gravité. Par ailleurs, en absence de contre-indication, le traitement curatif par blood patch dont l'efficacité est avérée, est maintenant assez systématiquement proposé aux personnes qui souffrent de ce type de complication. Les auteurs rapportent l'observation originale d'une patiente pusillanime dont la symptomatologie très invalidante consécutive à une EDA réalisée à l'âge de 31 ans lors d'une APD pour césarienne, a pu être définitivement soulagée par un blood patch qu'elle a fini par accepter presque huit ans plus tard. À l'occasion d'une brève revue de littérature, la physiopathologie et la prise en charge sont discutées.

Long lasting postural puncture headache secondary to an unintentional dural puncture (UDP) during epidural anaesthesia (EA) or following deliberate dural puncture for spinal anaesthesia, and neurological or neuroradiological examination, is becoming unusual. Placing the bevel of Tuohy needle parallel to the longitudinal axis of the dural cylinder when searching for epidural space, and in the later cases, widespread use of small diameter, atraumatic needles, have decreased both its incidence and severity. Furthermore, in the absence of any contraindication, patients suffering from such troubles are now quite systematically offered a curative epidural blood patch (EBP) whose effectiveness is documented. Authors report an original case of a fainthearted patient who underwent caesarean section under EA when she was 31-year-old, and whose disabling painful symptoms related to a UDP were successfully relieved by one EBP that she ended to accept more than seven years later. On the occasion of a short literature review, physiopathology and management are debated.
