Preventing complications, especially microangiopathy, in type 2 diabetes (T2D) requires glycemic control tailored to each patient according to individualized goals. About 20 to 25% of patients with T2D require insulin therapy and today it is often initially a basal long-acting insulin. These basal insulins, launched on the market for 10 years, are more stable allowing easier titration, and represented a significant step forward. Despite these new therapeutic tools, there is still a delay in the introduction and titration of these insulins on the part of patients and caregivers leading to results below those expected. A «L&Vous Charter» has been initially drafted to describe the criteria of good use of long-acting insulin in T2D. This second part is an Observatory conducted in real life, recruitment provided by the pharmacist (1129 patients with T2D on long-acting insulin) to describe the characteristics of these patients, their practices, their experiences, their autonomy, the respect of the charter, the success factors of satisfaction, or those deemed limiting. From the first lessons of this observatory, it appears that therapeutic education is not sufficiently available, and learnings on insulin titration are needed to promote metabolic control success, as well as autonomy of the patients. Involvement of the diabetologist appears as a favorable factor. This study is the largest carried out in France in this respect.
