The Insulin Detemir Algerian Study (IDEALS) is a national observational study assessing the safety and efficacy of insulin detemir in daily clinical practice, in Algeria. The cohort included 1974 patients with type 2 diabetes who failed to achieve a good glycemic control with oral antidiabetics and were treated with insulin detemir once daily at bedtime, with a follow-up done at weeks 12 (W12) and 26 (W26). The analysis of the results showed a significant improvement in glycemic control with a decrease in mean HbA1c at W12 (-0.63%; p < 0.0001) and at W26 (-0.86%; p < 0.0001). The number of hypoglycemic episodes decreased significantly, -96.8% at W26 vs. initially (p < 0.0001). There was a slight weight gain (+1.06 kg) at the end of follow-up. The data reported in this study conducted in real life condition corroborate those found in clinical trials and suggest that a more intensive titration of the insulin detemir dose which was, as a mean, of 21.7 U/day at the end of the follow-up period, could have led to a better glycemic control.
