The management of type 2 diabetes is a difficult task because of the complex pathophysiology of the disease, the poor patient's compliance to lifestyle changes, the increasing variety of available pharmacological agents, the controversies regarding glucose control targets and the need for implementing a multirisk cardiovascular prevention strategy. Even if general guidelines may be proposed, it is crucial to individualize the therapy according to patient's characteristics, with the aim to obtain the best benefit-risk ratio, at a reasonable cost. The present paper summarizes the new strategy in the management of type 2 diabetes after the recent publication of several large controlled clinical trials and the updated consensus statement of the American Diabetes Association (ADA) and the European Association for the Study of Diabetes (EASD). It emphasizes the crucial role of an individualized global strategy and of the therapeutic education of both health care providers and diabetic patients.
