L’enthèse : la piste pour la pathogénie des spondyloarthrites ?

RésuméLes spondyloarthrites constituent un groupe de manifestations dont la caractéristique commune est le développement d’une inflammation à l’interface os–tendons/ligaments ou os–capsule articulaire. Cette interface appelée « enthèse » peut être le siège d’une ossification dans la spondyloarthrite. Le développement des techniques d’imagerie fines comme l’IRM ont suscité un intérêt nouveau pour cette structure en définissant mieux les différentes séquences menant à l’ossification de l’enthèse et en montrant en particulier l’importance d’une inflammation initiale et d’une métaplasie graisseuse avant l’apparition de cette ossification. Les phénomènes physiopathogéniques conduisant à l’apparition primitive de l’inflammation semblent être d’origines diverses, liés au stress mécanique, par exemple, ou à la présence de cellules résidantes sensibles à l’interleukine 23 et productrices de cytokines pro-inflammatoires. La compréhension des mécanismes pathogéniques à l’origine de l’inflammation et de l’ossification de l’enthèse pourrait ainsi être une piste intéressante pour expliquer la pathogénie des SpA.

The spondyloarthritis are a group of chronic inflammatory conditions whose common characteristic is the development of inflammation in the bone interface with tendons/ligaments or joint capsule. This interface, called “enthesitis”, may be the seat of ossification in spondyloarthritis. The development of fine imaging techniques such as MRI have attracted new interest in this structure by better defining the sequences leading to the ossification and showing in particular the importance of initial inflammation and fatty metaplasia before the appearance of this ossification. The physiopathological phenomena leading to the early onset of inflammation appears to be of various origins, related to mechanical stress, for example, or the presence of resident cells that are responsive to interleukin 23 and produce pro-inflammatory cytokines. Understanding of the pathogenic mechanisms underlying inflammation and ossification of enthesis could well be the clue to SpA pathogenesis.
