Tratamiento de la hipertensión arterial en situaciones especiales

ResumenLos pacientes hipertensos padecen con mucha frecuencia situaciones clínicas y patologías asociadas que pueden influir en la elección del tratamiento más seguro y eficaz. El médico debe tener en cuenta estas características por diferentes motivos. En primer lugar, la efectividad de los tratamientos está influenciada por estas asociaciones, haciendo aconsejable la utilización preferente de un determinado fármaco. En segundo lugar, hay medicamentos que están contraindicados en hipertensos con enfermedades concretas. Finalmente, en muchas ocasiones los pacientes toman fármacos por sus patologías que pueden interaccionar con los hipotensores, modificando su eficacia o aumentando su toxicidad. En esta actualización repasaremos el tratamiento de los pacientes hipertensos con situaciones especiales y con algunas patologías asociadas que son muy prevalentes.

Hypertensive patients have very often clinical situations and comorbidities that may influence the choice of the most safe and effective treatment. The physician should be aware of these characteristics for different reasons. First, the effectiveness of treatments is influenced by these associations, making wise the preferential use of a particular drug. Secondly, there are medications that are contraindicated in hypertensive patients with specific diseases. Finally, patients often take their drugs for diseases that can interact with hypotensive altering their efficacy or increasing toxicity. In this update we review the treatment of hypertensive patients with special situations and some associated diseases that are highly prevalent.
