Insuficiencia cardiaca aguda

ResumenLa insuficiencia cardiaca es una enfermedad cuya prevalencia está en aumento. Es la causa más frecuente de ingreso hospitalario en la población anciana. En los servicios de Urgencias la insuficiencia cardiaca puede aparecer como una situación nueva o como una agudización de un proceso ya existente. Existe un perfil de paciente en el que el diagnóstico será sencillo; sin embargo, cuando los síntomas más típicos están ausentes el diagnóstico es un reto para el clínico. Siempre debemos mantener un alto nivel de sospecha y utilizar las herramientas en nuestra mano, como los péptidos natriuréticos y la ecografía cardiaca y pulmonar, que sí han revolucionado el diagnóstico de la insuficiencia cardiaca en los servicios de Urgencias. El tratamiento de la insuficiencia cardiaca aguda ha variado poco en los últimos años. Se han estudiado muchos fármacos nuevos que, si bien han encontrado su sitio en situaciones muy específicas, no han cambiado significativamente el cuadro de la insuficiencia cardiaca aguda.

Heart failure is a disease whose prevalence is increasing. It is the most frequent cause of hospitalization in the elderly population. In the Emergency Department heart failure it may appear as a new situation or a worsening of an existing process. There is a profile of patients in which the diagnosis is simple; however, when the most typical symptoms are absent diagnosis is a challenge for clinicians. We must always maintain a high level of suspicion and use the tools in our hands as natriuretic peptides and the cardiac ultrasound and lung that have actually revolutionized the diagnosis of heart failure in the emergency services. Treatment of acute heart failure has changed little in recent years. We have studied many new drugs that have found their place in very specific situations but have not significantly changed the picture of acute heart failure.
