Microenvironnement du cancer de la prostate : structure, fonctions et applications thérapeutiques

RésuméIntroductionDans le domaine du cancer de la prostate, de plus en plus d’études tendent à démontrer le rôle prépondérant de la zone située entre la tumeur et l’hôte : le microenvironnement tumoral. Le but de cette revue est de décrire la structure et les fonctions du microenvironnement du cancer de la prostate, ainsi que les principaux traitements le ciblant.Matériel et méthodesLes bases de données PubMed et ScienceDirect ont été interrogées en associant les mots clés « tumor microenvironment » et « neoplasm therapy » ainsi que « microenvironnement tumoral » et « traitements ». Sur les 593 articles initialement retrouvés, 50 ont finalement été retenus.RésultatsLe microenvironnement tumoral comprend principalement des éléments de l’hôte détournés de leur fonction primaire afin de favoriser le développement tumoral. On y trouve des cellules immunitaires, un tissu de soutien, ainsi qu’une néovascularisation sanguine et lymphatique. La mise en évidence du rôle majeur du microenvironnement a conduit à la mise au point de traitements ciblés, notamment les anti-angiogéniques et les traitements immunomodulateurs.ConclusionMicroenvironnement tumoral, tumeur et hôte s’influencent mutuellement et créent une situation variable dans le temps. L’amélioration des connaissances du microenvironnement tumoral prostatique permet peu à peu le passage d’une approche « tumoro-centrique » à celle plus globale d’« écosystème tumoral ». Cette démarche a permis l’émergence de nouveaux traitements dont la place dans l’arsenal thérapeutique reste à déterminer.

SummaryIntroductionIn the field of prostate cancer there is a growing tendency for more and more studies to emphasise the predominant role of the zone situated between the tumour and the host: the tumour microenvironment. The aim of this article is to describe the structure and the functions of the prostate cancer microenvironment as well as the principal treatments that are being applied to it.Material and methodsPubMed and ScienceDirect databases have been interrogated using the association of keywords “tumour microenvironment” and “neoplasm therapy” along with “microenvironnement tumoral” and “traitements”. Of the 593 articles initially found, 50 were finally included.ResultsThe tumour microenvironment principally includes host elements that are diverted from their primary functions and encourage the development of the tumour. In it we find immunity cells, support tissue as well as vascular and lymphatic neovascularization. Highlighting the major role played by this microenvironment has led to the development of specific treatments, notably antiangiogenic therapy and immunotherapy.ConclusionThe tumour microenvironment, the tumour and the host influence themselves mutually and create a variable situation over time. Improvement of the knowledge of the prostate cancer microenvironment gradually enables us to pass from an approach centred on the tumour to a broader approach to the whole tumoral ecosystem. This enabled the emergence of new treatments whose place in the therapeutic arsenal still need to be found.
