Histerectomía por mioma: ¿un asunto de anemia y tamaño uterino?

ResumenObjetivoUna histerectomía por mioma uterino se decide habitualmente por la sintomatología y el tamaño del útero. El objetivo de este estudio es determinar si hay variables objetivas útiles para evaluar la indicación de histerectomía.Sujetos y métodosHemos revisado 103 casos de histerectomías realizadas consecutivamente en nuestro hospital. Las variables recogidas para el estudio han sido la edad, el peso de la pieza quirúrgica y la cifra de hemoglobina preoperatoria.ResultadosEl 87,4% de las pacientes (intervalo de confianza del 95%, 81% a 94%) tenían anemia moderada o severa y/o un peso del útero superior a 280 g. Hemos encontrado diferencias significativas tanto en las cifras de hemoglobina como en el peso del útero, al considerar la edad de las pacientes.ConclusionesLas tasas de histerectomía son muy variables, pero las cifras de hemoglobina preoperatoria y el peso de la pieza quirúrgica pueden introducir objetividad en las indicaciones.

ObjectiveThe decision to perform hysterectomy for uterine myoma is usually based on the patient's symptoms and uterine size. The aim of this study was to determine if there are any objective variables that could be useful for assessing the indication for hysterectomy.Subjects and methodsWe reviewed 103 cases of hysterectomies consecutively performed in our hospital. The following variables were analyzed: age, weight of the surgical specimen, and preoperative hemoglobin level.ResultsModerate or severe anemia and/or a uterus weight over 280 g was found in 87.4% of the patients (95% confidence interval, 81-94%). There were significant differences in both hemoglobin level and uterine weight, taking into account patient age.ConclusionsHysterectomy rates vary widely, but preoperative hemoglobin levels and the weight of the surgical specimen should be considered for greater objectivity in decision-making.
