Fungal keratitis in an intertropical area: diagnosis and treatment problems. Advantage of local use of polyvidone iodineFungal infection is rarely investigated in keratitis. The authors report five cases of fungal keratitis observed at the le Dantec University Teaching Hospital of Dakar, Senegal, involving two males and three females. Diagnosis was made by examination of smears and cultures of corneal scrapings revealing Candida albicans isolated in four patients and Acremonium strictium in another after 2 or 12Â weeks of treatment with antibiotics, antivirals, or steroids. Povidone iodine 2.3% concentrated eye drop was used alone or with an azole for 4 a mean of weeks. All patients presented corneal scars. Fungal keratitis must be considered in presence of torpid corneal ulcer and corneal scraping must be systematically done. Topical povidone iodine alone or associated with azole may be an alternative fungal keratitis treatment in intertropical areas.

Les auteurs rapportent cinq cas de kératite mycosique observés au Centre Hospitalier Universitaire de Dakar chez deux hommes et trois femmes. Le diagnostic fut établi après deux à douze semaines de traitement antibiotique par voie locale et générale, antiviral ou stéroïdien par un examen microscopique des produits de curetage des lésions cornéennes. Un Candida albicans fut isolé chez quatre patients, et un patient était infecté par un Acremonium strictium. La polividone iodée à la concentration de 2,3 % en collyre seule ou associée au kétoconazole fut utilisée pendant quatre semaines en moyenne. Tous les patients présentèrent des cicatrices cornéennes. La kératite mycosique doit être recherchée devant tout ulcère torpide, et le prélèvement doit être systématique. La polividone en topique associée à un azolé peut être une alternative au traitement antifongique en zone intertropicale.
