Confabulations fantastiques ou délire ?

RésuméIntroductionLes confabulations sont des propos non pertinents, classiquement observés chez des patients amnésiques. Les confabulations spontanées fantastiques sont rares et difficiles à interpréter en tant que confabulation ou propos délirant.MéthodesNous rapportons le cas de deux patients présentant des confabulations spontanées fantastiques après lésion cérébrale. Nous cherchons à discuter la nosologie de ce trouble, neurologique ou psychiatrique.RésultatsMme H. présenta des confabulations fantastiques dans les suites d’une rupture d’anévrisme du tronc basilaire. Elle présentait alors un syndrome amnésique et dysexécutif. M. U. présenta des propos spontanés et incohérents après la survenue d’une anoxie cérébrale par arrêt cardiaque. Il présentait un syndrome amnésique et un syndrome dysexécutif. Les deux patients présentaient une adhésion complète aux propos totalement improbables et fantastiques et agissaient en fonction des ces confabulations.DiscussionLes explications neuropsychologiques avancées dans la littérature pour expliquer les confabulations sont au nombre de deux : le défaut d’encodage temporel de Schnider et le trouble de contrôle stratégique lors de la récupération de souvenirs. Or, ces explications ne rendent pas compte de confabulations fantastiques qui ne sont pas de simples « vrais » souvenirs rappelés de façon non pertinente. En revanche, le tableau clinique et l’évolution spontanée de ces patients rentrent dans le cadre nosographique du délire tel qu’il est défini dans les classifications internationales.ConclusionIl semble donc que les confabulations fantastiques ne peuvent répondre de façon satisfaisante aux définitions purement neurologiques des confabulations spontanées.

IntroductionConfabulations are inaccurate narratives purporting to convey information about the world or the self. Fantastic utterances from brain-injured patients are difficult to interpret as confabulation or delusion.MethodsStarting from two clinical observations, we analyse the terms “spontaneous confabulations” and “delusion”.ResultsTwo brain-injured patients presented with fantastic utterances; the adhesion was complete for both patients. They also suffered from amnesic and executive disorders. Clinical interviews with neuropsychologist and psychopathologist could not distinct between spontaneaous confabulations and delusion.DiscussionNeuropsychological interpretations of spontaneous confabulations are (1) the deficient strategic retrieval process arising from executive failures and (2) confusion of the temporal order of information within memory. These hypotheses cannot explain the fantastic utterances described. Are spontaneous fantastic confabulations a specific entity or did these patients present a morbid association of neurological and psychiatric disorders? Fantastic confabulations are very scarcely described and the association of neurological and psychiatric impairments enables us to better describe the clinical observations. Spontaneous confabulations reported after amnesic and executive disorders are indeed typically composed of the patient's past experiences and do not usually comprehend fantastic utterances.ConclusionNeurological and psychological analyses of the fantastic utterances from two brain-injured patients do not respond to the definition of spontaneous confabulation. Fantastic confabulations are a rare entity and we propose another explanation consisting of the association of neurological and psychiatric disorders.
