Syndrome du canal carpien et poignet à ressaut révélant un fibrome de la gaine des fléchisseurs

RésuméLe fibrome de la gaine tendineuse (FGT) est une tumeur bénigne rare, exceptionnellement responsable d’un syndrome du canal carpien et (en même temps) d’un poignet à ressaut : nous avons trouvé cette « double association » moins de dix fois dans la littérature anglo-saxonne et francophone. Nous rapportons l’observation d’un patient de 63 ans, menuisier et droitier qui présentait un claquement du poignet droit au cours de la flexion-extension en plus d’un syndrome du canal carpien, secondaires à un fibrome de la gaine du flexor digitorum superficialis au poignet droit. L’imagerie (échographie et IRM) a permis de suspecter le diagnostic et la biopsie-exérèse l’a confirmé. Au dernier recul (1 an), nous avons noté une disparition de la symptomatologie sans signes de récidive.

The tendinous sheath fibroma (TSF) is a rare benign tumor, exceptionally responsible for carpal tunnel syndrome and “trigger” wrist: we found this association less than ten times in the English and French literature. We report the case of a 63-year-old right-handed carpenter who featured a triggering phenomenon of the right wrist during the flexion-extension movements and compression of the median nerve at the carpal tunnel, secondary to a TSF of the flexor digitorum superficialis. The diagnosis was suspected at the sonography and MRI, the tumor was excised and proven histologically to be a TSF. One year later, the patient remained free of symptoms.
