Acrométastase d’un carcinome urothélial de la vessie : à propos d’un cas et revue de la littérature

RésuméLes auteurs rapportent un nouveau cas d’acrométastase de la main, d’un carcinome urothélial de la vessie, chez une patiente âgée de 46 ans, aux antécédents de lobectomie inférieure gauche pour adénocarcinome bronchique. L’examen clinique retrouvait une tumeur dorsale de la main, inflammatoire et douloureuse. La radiographie montrait une ostéolyse mal limitée de la base du quatrième métacarpien. Deux études immuno-histochimiques ont été nécessaires pour confirmer l’origine vésicale de cette métastase. Les auteurs proposent une revue de la littérature abordant la difficulté du diagnostic, du traitement et du sombre pronostic de ces lésions.

The authors report a new case of acrometastasis of the hand, from a urothelial carcinoma of the bladder, in a 46-year-old woman, with a history of left lower lobectomy for bronchial carcinoma. Physical examination revealed a tumor of the dorsal hand, inflammatory and painful. The radiograph showed ill-defined osteolysis of the base of the fourth metacarpal. Two immunohistochemical studies were needed to confirm the origin of the bladder metastasis. The authors propose a review of the literature addressing the difficulty of diagnosis, treatment and poor prognosis of these lesions.
