« Un cinquième métacarpien flottant ». À propos d’un cas

RésuméLes auteurs rapportent l’association d’une fracture-luxation proximale du cinquième métacarpien avec une luxation métacarpophalangienne (cinquième métacarpien flottant) et d’une fracture du quatrième métacarpien. La lésion décrite était la résultante d’un choc direct au niveau du bord ulnaire de la main et d’une hyperextension forcée de la première phalange. Le traitement a consisté en une réduction orthopédique associée à un brochage percutané carpométacarpien et inter-métacarpien. À 12 mois, le résultat fonctionnel était acceptable.

We present the case of a patient with a fracture-dislocation of the fifth metacarpal and a dislocation of the metacarpophalangeal joint (floating fifth metacarpal) combined with a fracture of the fourth metacarpal. The mechanism of injury and management of this uncommon injury is reported. The “floating fifth metacarpal” and the fourth metacarpal fracture were reduced by closed techniques with percutaneous fixation. Twelve months later, the patient had a full range of wrist and finger movement, he was pain-free and without any residual disability.
