Centralisation progressive de la main dans la main bote radiale congénitale

RésuméL’objectif thérapeutique d’une main bote radiale congénitale est d’avoir en fin de croissance une main fonctionnelle, c’est-à-dire une main avec un pouce permettant la pince, dans l’axe de l’avant-bras avec un aspect le plus esthétique possible. Le challenge réside surtout dans l’obtention d’une main située dans l’axe de l’avant-bras. Certains réalisent ainsi une centralisation ou une radialisation chirurgicale par libération directe de l’ensemble des tissus mous rétractés. Il est aussi possible de corriger la main sans geste chirurgical invasif, en mettant en place un fixateur externe et en corrigeant la déformation de façon progressive. Le fixateur externe Taylor Spatial Frame™ est un système de fixation externe qui permet, à l’aide d’un logiciel informatique, de réaliser cette correction de façon précise.

The therapeutic objective of a congenital radial clubhand is to obtain, at the end of the growth, a functional hand, that is a hand with a thumb, in the axis of the forearm with the most cosmetic aspect as possible. The challenge lies in the fact that the hand is not in the axis of the forearm. Thus, some authors realize a surgical centralization or radialization by doing a direct release of the retracted soft tissues. On the other hand, it is possible to correct the hand without any invasive surgery by using an external fixator allowing to correct the hand progressively.
