Talar injuries present a unique set of challenges because of the precarious blood supply and its special function. Restoring anatomy is extremely important but does not necessarily exclude poor outcome because avascular necrosis, malunion, and posttraumatic arthritis are common sequelae. The reconstructive options for avascular necrosis and posttraumatic arthritis are numerous. Anatomic reduction of varus malunions is an effective treatment in the face of normal adjacent joints.
