Proximodistal interphalangeal arthrodesis of the little finger: A series of 7 cases

As an alternative to amputation of the little finger, we report here seven cases of shortening arthrodesis carried out by resecting the middle phalanx and proximodistal interphalangeal (PDIP) arthrodesis. Our cohort consisted of 6 males and 1 female (58 years average age), with a stiff little finger secondary to Dupuytren's disease or trauma. All fingers were approached dorsally; after resection of the middle phalanx and decortication of the subchondral bone, fusion of the remaining phalanges was performed using an intramedullary self-breaking screw-pin. At a mean follow-up of 34.9 months, pain decreased significantly (1.4/10 versus 5.4/100 preoperatively), the QuickDASH score improved significantly (33/100 versus 51/100 preoperatively) and all the joints had fused. One patient suffered from cold intolerance. PDIP arthrodesis is an alternative salvage procedure to amputation for multioperated stiff little fingers that does not burn any bridges if it fails.

RésuméParmi les alternatives à l’amputation digitale de l’auriculaire, nous rapportons ici 7 cas d’arthrodèse accourcissante par résection de la phalange moyenne ou arthrodèse interphalangienne proximo-distale (IPPD). Il s’agissait de 6 hommes et 1 femme, âgés en moyenne de 58 ans, présentant un auriculaire enraidi suite à une maladie de Dupuytren ou à un traumatisme. Tous les patients ont été opérés par voie dorsale ; après excision de la phalange moyenne et avivement des surfaces articulaires en contact, une arthrodèse par vis-broche à compression autocassante centromédullaire des deux phalanges restantes a été réalisée. Au recul moyen de 34,9 mois, la douleur était significativement moindre (1,4/10 en postopératoire vs 5,4/10 en préopératoire) Le score QuickDASH était significativement amélioré (33/100 en postopératoire vs 51/100 en préopératoire). Toutes les arthrodèses IPPD ont consolidé. Un patient présentait des douleurs au froid. L’arthrodèse IPPD est une solution de sauvetage alternative à l’amputation, qui doit être réservée à des doigts raides et multiopérés ; elle ne coupe pas les ponts en cas d’échec.
