Unicompartmental knee arthroplasty has recently enjoyed a resurgence in popularity. Small incision surgical techniques have evolved that allow a rapid return to activity with minimal morbidity. Patients with isolated one-compartment disease and no associated ligamentous instability are good candidates for the limited arthroplasty procedure. Quadriceps-sparing instruments greatly facilitate the procedure by easing the muscle preserving approach and allowing a seamless transition from unicompartmental to total knee arthroplasty if necessary. The use of a single instrument design also facilitates operating room efficiency and improves team familiarity with instrumentation. A total of 343 unicompartmental knee arthroplasties have been performed during the last 4 years, with 5 revision surgeries (1.5%). Excellent knee function and high patient satisfaction combine with expected long-term durability to recommend this procedure.
