(iv) Hemiarthroplasty in proximal humeral fractures: surgical technique

Proximal humeral fractures are common and increasing in incidence. The majority of undisplaced injuries can be treated conservatively but the optimal management of displaced three and four part fractures remains debatable. Hemiarthroplasty is typically used for fractures that cannot be reliably reconstructed using locking plates or where there is evidence that the blood supply to the head has been significantly compromised.This article focuses on the surgical technique of hemiarthroplasty and highlights the key principles necessary to achieve an optimal outcome. Finally, the option of reverse shoulder arthroplasty for the treatment of proximal humeral fractures is briefly discussed.
