Valoración clínico-funcional tras tratamiento con ácido hialurónico y onda corta en pacientes con artrosis de rodilla

ResumenIntroducciónAlgunas terapias aplicadas en la gonalgia de causa degenerativa han demostrado efectividad y están incluidas en protocolos, sin embargo, la comparación de los tratamientos aplicados con onda corta (OC) y ácido hialurónico (AH) en nuestro medio no ha sido evaluada. Se pretende conocer y comparar los beneficios terapéuticos obtenidos en pacientes con gonartrosis y dolor sometidos a estas dos terapias.Material y métodoSe trataron 72 rodillas de 56 pacientes divididos en dos grupos de 36. A los del primer grupo se les aplicaron 5 infiltraciones de AH, a los del segundo 20 sesiones de OC. Se utilizó la “t” de Student para comparar niveles previos y posteriores de la escala visual analógica (EVA) e índice de Lequesne en ambos grupos (y su diferencia), y para la valoración subjetiva del paciente se utilizó la prueba de χ2, con un nivel de significación del 5%.ResultadosEl descenso medio en la EVA fue: (AH 2,6 ± 2) (OC 0,7 ± 1,8) con p < 0,001, y en el índice de Lequesne: (AH 4,3 ± 2,5) (OC 2,2 ± 2,3) con p = 0,001. La valoración subjetiva tras el tratamiento: AH [igual 4 (11,8 %), mejor 29 (85,3 %)], OC [igual 17 (47,2 %), mejor 18 (50 %)]. El coeficiente de correlación entre el descenso en EVA y el índice de Lequesne fue elevado, siendo mayor dentro del grupo de OC (0,61).ConclusiónSe confirma un mayor descenso del dolor y una mejoría funcional en los pacientes tratados con AH con respecto a los tratados con electroterapia. Se aporta como novedad la existencia de una buena correlación entre la EVA y el índice de Lequesne.

The purpose of this study is to ascertain and compare the therapeutic benefits obtained in patients with gonarthrosis and pain subjected to these two therapies.Patients and methods72 knees from 56 subjects were treated. The subjective evaluation after treatment was: HA = [equal 4 (11.8 %), improvement 29 (85.3 %)], SW = [equal 17 (47.2 %), improvement 18 (50 %)]. Correlation coefficient between the decrease on the visual analogue scale and the one on the Lequesne score was significant, especially in the short wave group (0.61).DiscussionWe confirmed greater pain reduction and better functional improvement in patients treated with hyaluronic acid than those treated with electrotherapy. A significant correlation between the Lequesne score and the visual analogue scale was found.
