Immunogénicité et réactogénicité des vaccins DTCa-P-HepB/Hib et DTCa-P/Hib coadministrés avec le vaccin pneumococcique osidique conjugué, adsorbé, chez des nourrissons en primovaccination et en rappel selon le calendrier vaccinal français

RésuméObjectifÉvaluer l’immunogénicité et la réactogénicité des vaccins combinés DTCa-P/Haemophilus influenzae (Hib) (Infanrixquinta®) et DTCa-P-HepB/Hib (Infanrixhexa®) coadministrés chez les nourrissons avec le vaccin pneumococcique osidique conjugué heptavalent adsorbé (Prevenar®) conformément aux recommandations vaccinales françaises.MéthodeÉtude multicentrique, non contrôlée, en ouvert, conduite en France chez des nourrissons recevant 1 dose du vaccin combiné DTCa-P-HepB/Hib à 2, 4 et 16–18 mois coadministrée avec le vaccin pneumococcique conjugué et 1 dose du vaccin combiné DTCa-P/Hib à 3 mois également coadministrée avec 1 dose de vaccin pneumococcique conjugué.RésultatsParmi les 102 nourrissons inclus dans l’étude, 64 ont été analysés (âge moyen à l’inclusion de 1009 semaines ± 1,22 ; garçons : 58 %) dans la population per protocole (PP) d’immunogénicité. Un mois après la dose de rappel du vaccin DTCa-P-HepB/Hib, 93,8 % des nourrissons ont développé des concentrations protectrices en anticorps anti-HBs supérieur ou égal à 10 mUI/ml. Le taux de séroprotection vis-à-vis du composant H. influenzae type b (taux d’anticorps anti-PRP ≥ 0,15 μg/ml) était de 98,4 % et la réponse immunitaire obtenue pour les 7 sérotypes pneumococciques (taux d’anticorps antipneumococciques ≥ 0,05 μg/ml) variait de 98,4 à 100 %. La réactogénicité locale augmentait avec le nombre de doses administrées, mais était comparable entre les vaccins combinés et le vaccin pneumococcique conjugué. L’incidence de la fièvre augmentait entre la primovaccination et le rappel.ConclusionLe profil d’immunogénicité et de réactogénicité des vaccins combinés DTCa-P-HepB/Hib et DTCa-P/Hib coadministrés avec le vaccin pneumococcique conjugué, selon le schéma recommandé dans le calendrier vaccinal français, est satisfaisant et comparable à ceux précédemment rapportés.

SummaryObjectiveAssessment of immunogenicity and reactogenicity of DTPa-IPV/Hib (Infanrixquinta®) and DTPa-IPV-HBV/Hib (Infanrixhexa®) combined vaccines coadministered in healthy infants with 7-valent pneumococcal conjugate vaccine (Prevenar®), according to the current French vaccine recommendations.MethodMulticenter, open label, non-controlled study performed in France. Each subject received 1 dose of DTPa-IPV-HBV/Hib combined vaccine at 2, 4 and 16–18 months of age, coadministered with conjugated pneumococcal vaccine and 1 dose of DTPa-IPV/Hib vaccine at 3 months of age coadministered with conjugated pneumococcal vaccine.ResultsAmong the 102 enrolled infants, 64 were analysed (10.09 weeks ± 1.22 of age; boys: 58%) in the According-To-Protocol (ATP) immunogenicity cohort. One month after the administration of the booster dose of DTPa-IPV-HBV/Hib vaccine, 93.8% of subjects had protective titres for anti-HBs antibody superior or equal to 10 mIU/ml (primary objective). Seroprotection rate against Haemophilus influenzae type b component (anti-PRP antibody ≥ 0.15 μg/ml) was 98.4% and the immune response for the 7-valent pneumococcal serotypes (antipneumococcal antibody ≥ 0,05 μg/ml) was between 98.4 and 100%. Local reactogenicity increased with the number of doses administered, but was comparable between combined vaccines and conjugated pneumococcal vaccine. The incidence of fever increased between the primary vaccination and the booster.ConclusionThe immunogenicity and reactogenicity profile of DTPa-IPV-HBV/Hib and DTPa-IPV/Hib combined vaccines coadministered with conjugated pneumococcal vaccine according to the schedule recommended in the French vaccine calendar is acceptable and similar to previous reports.
