Bronchoscopie interventionnelle en pédiatrie

RésuméLes indications de bronchoscopie interventionnelle sont multiples chez l’enfant : extraction de corps étranger, broncho-aspiration, intubation difficile, intubation sélective et contrôle des voies aériennes en cours de chirurgie du thorax ou chez l’enfant intubé en réanimation. Certaines techniques de bronchoscopie interventionnelle empruntées à l’adulte peuvent trouver des indications en pédiatrie : dilatation de sténose, photorésection au laser, pose de stent trachéo-bronchique. Il n’existe aucune règle concernant l’application de telles techniques en pédiatrie et dont le bénéfice attendu doit être comparé à celui de techniques plus conventionnelles.

There is a wide range of indications for therapeutic bronchoscopy in children today: foreign body removal, bronchoaspiration, endoscopy-assisted tracheal intubation, selective intubation and airway management during thoracic surgery or in children undergoing mechanical ventilation. Some adult-derived methods may find potential indications in pediatric patients: airway stenosis dilation, laser photoresection, tracheobronchial stenting. There are no rules regarding such procedures in children, and supposed benefits have to be weighted against those of more conventional therapies.
